We welcome referrals from general practitioners, medical specialists, and other healthcare professionals. Our practice is committed to providing carefully considered, evidence-based care within a structured and professional framework.
Referral Process
Patients can be referred by submitting a referral letter, preferably via ZorgDomein. Alternatively, patients may register themselves through the patient portal on our website; this can also be done in collaboration with the referring professional.
Following registration, we aim to contact the patient within 24 hours to schedule an initial intake consultation.
Referral letters should be addressed to Cristina Zwaan Practice for Psychiatry and Psychotherapy and include:
- Patient identification and contact details
- Date of referral
- Referrer's name and personal AGB code
- A concise description of the presenting complaints, including a provisional DSM-5 diagnosis
- Indication for specialist mental healthcare (SGGZ)
- Information on whether this concerns a re-referral

Diagnostic Approach
We do not offer standalone diagnostic assessments, particularly in the context of Attention Deficit Hyperactivity Disorder, without a corresponding treatment indication.
Our approach is grounded in a comprehensive psychiatric and psychological assessment, focusing not only on classification but on the underlying processes that shape functioning in daily life. Diagnosis is therefore always embedded within a broader treatment trajectory.
For patients seeking assessment only, we recommend contacting the patient's health insurer for care mediation or referring back to the general practitioner for alternative options.
Eating Disorders
A safe and responsible start of treatment for patients with an eating disorder requires an up-to-date somatic assessment. As this cannot be conducted within our practice, we request that the referring physician carries out the necessary medical evaluation, including laboratory testing, and shares the findings with us prior to the start of care.
